Clontarf Dental Practice Limited is a limited liability company registered in Ireland that trades under the business name “&Alex” (“Our Company”). Our Company is affiliated with another limited liability company registered in Ireland, Heathwood Limited. Our Company collects the following data:

  • Personal identification information (name, date of birth, PPS number, email address, phone number, etc.)
  • Demographic information such as profession / place of work
  • Data concerning your health including:
  • your medical history;
  • your dental care history;
  • your dental care and health;
  • clinical notes;
  • payment information; and
  • appointment information.

2. How do we collect and use your data and why?

You directly provide Our Company with most of the data we collect. We collect data and process data when you:

  • Become (or make an inquiry about becoming) a patient of the practice and continue to collect it while you are a patient of the practice.
  • Check with us whether you are entitled to any financial contribution from the State for your dental care via your PRSI contributions (for this purpose we collect your date of birth and your PPS number).
  • Voluntarily complete a customer survey or provide feedback.
  • Use or view our website via your browser’s cookies.

Our Company may also receive your data indirectly from other dental practices or healthcare professionals.

The legal basis for the processing of your personal data is that processing is necessary for the performance of a contract to which you are a party. Without processing this personal data we would not be able to provide you with the dental care services for which you have engaged us.

  • it is in ours and your legitimate interests to have the data processed to check whether you are entitled to any benefit; and
  • we obtain your consent to the said processing prior to same on the relevant web page.

We will retain personal data for as long as required for the purposes set out above or as long as required to comply with any legal obligations to which we are subject.

  • to provide you with updates about offers or our products and newsletters to which you have subscribed;

We share this data with our practice management system provider. We may also send you emails through our email service provider. They may only process this data for the purpose of providing us with their services, and no other purpose. Our Company would like to send you information about products and services of ours that we think you might like, as well as those of our partner companies.
